I think this picture has been altered (PhotoShop?). Notice the differences in the base collar tab color. One is more pronounced (darker) than the other. And the wearing of a runes tab and a TK tab on the rank side in combination would not, I'm sure, have been tolerated.
Is the source and location where this photo was taken known?

Just because it looked cool...
The following image has been floating around on several forums...
(not sure who to give credit to...)
Here's a Flemish volunteer who picked up a Norwegian tab lion in metal (which resembled a Flemish lion but included an axe),
a gift from a Norwegian volunteer, who he met while staying at a hospital, so he decided to stick it on his blank collar tab...

Two scenarios with that example:
An SS member of the Totenkopf Division who received a regular (replacement) SS uniform with the SS tab factory applied and converted it to TK by adding the hat skull
or he was reassigned from another unit to TK and made the modification for that reason. This example is somewhat within regulation as the rank tab was not decorated.
Other examples were more extreme!
The bottom line is that these guys did whatever in their eyes looked cool.
Here 2 examples of Estonian Wehrmacht members who both placed a Baltic made metal SS cap eagles (note smaller size, a local production pattern)
pinned on top of a Heer trap ! Photo credit goes to "Otofoto" as marked

I have seen metal skulls on the collar of Freikorps and Wehrmacht soldiers for example, but it was the first time that I have seen it on SS collar tabs.
