Agree with Morten, I found a similiar one in Normandy two weeks ago, mine have the black eye and the red tongueCollector of Kriegsmarine and Küstenartillerie items

That is why I asked the question in the first place. All KM medical patches in my collection have no black eye or red tongue. A quick look on the web showed medical patches with black eye and red tongue as Heer patches, including grey patches.
See scan. The two patches at left are described as Heer while the two at right which are part of my collection have been described as KM. Did KM medical patches also have black eye and red tongue?
