The companion images to your image showing the replacement of the Maybach HL210 engine in “Tiger” №141 (Pz.Kpfw. VI Ausf. (E) 1st company of the 501st heavy tank battalion (s.Pz.Abt.501)
I believe its just a bad negative as the other images clearly show no insignia on the uniform shoulders for this crew.Attached Files

These are part of a Bundesarchiv series and generally their wartime negatives suffer from scratches, overdeveloped or underdeveloped areas, white spots and are not always in the best of condition given the circumstances of enduring WWII.
Here you can see there is no patch. Just an overdeveloped area among the other imperfections.Attached Files
