Here it is. Its from the hospital page of a Soldbuch.
The soldier was admitted from 1 November till 16 November 1944. The other entries above and below are from other times.
I'm trying to find out at what HVP, as i'm not sure with which unit he was at the time. Could be east- or westfront.
