If I understand you right, you mean why is the medal attached to the button hole...
I suppose the soldier wanted to present it they way EK2 and KVK2 were worn the day they have been awarded.
As the East Medal was not awarded in a ceremony, by attaching it to the uniform, this way of wearing it is rare to find on photos...
