Translation Help Please.

I can usually figure out sword mei but this sort of thing is beyond me at this stage. Can anyone assist?

The tag came from the Maizuru Naval depot. The family name has me stumped. I want to say Kuroda, but I couldn't say for sure.

I was told owners name was Tokita Tadashi? Any other translation help would be greatly appreciated.

鴇田 can be read several ways, including Tokida, Shimada, Houda, and more. Just googling it and doing a quick survey shows Tokida is the more common reading.
I suppose the given name could be Tadashi 忠, little hard to make out the bottom part but seems to match.
