This is a Japanese soldier's manual during World War II. The troop designation is the supplementary team of the ninth regiment of the 16th Division Infantry. Your soldier's name was Okamura Kisaburō (or Yoshisaburō). His given name implies he was the third male child born into his family (三郎 Saburō; i.e., Third son); and his grandfather (or father) wanted him to be a 喜 rejoice/pleasure.

Showa 14th Year [1939], December 23rd vicinity "Old Tiger" [Chinese place name??] in combat vicinity of Suginoki * Group killed in action from being pierced through by bayonet at the left chest area and left shoulder blade area.
After he died from the bayonet wounds (as Tom points out) he was promoted posthumusly to army corporal:

=Penetrating gun wound at left breast and left scapular region
銃創 is gun wound, not wound by bayonet.（not 銃剣）
