    Actually let me correct myself after studying a similer BD in my collection,
    the neck lining is a standard feature on these (against chafing)
    Its a really interesting intriguing piece, based on officers rank and medical insignia it probably belonged to a doctor. Maybe that's why the rank is in the neck so he can be identified fast.
