What kind of SS form is this?

Can anyone tell what this form was used for? Is it part of the Waffen SS application and verification process?

It reminds me of the U.S. military "DD214" form, which is essentially, your military record in a nutshell. Has his name, Frank First, and mother's name and address. Looks like his duty stations are stamped in there. In short order, one of the experts here at WAF will likely confirm or elaborate.

Uberwachungskarte ... SS personel file control card.
Records of the SS are

Basic record card (SS Stammkarte) that follows the soldier from unit to unit, like the Wehrpass but this control card was kept by the unit administration people.

I don't know about the originality.
