Original Italian wool and the liner material looks credible. The size stamp is a bit blue and blurry for me, but is possible. The trap seems sewn low for a two button, and when I have seen them low like this it's to get a resewn trap away from the thick top seam.

As I recall, some Italian wool cap manufacturers set the traps high and others set them low. The buttons are to the left and right of center.
Higher set traps are nice to find because as Robert pointed out, they are difficult reapply in that spot. Many of the high set SS traps have an extra stitch line across the top that runs past the sides of the trap as well.
IMO, if I saw puckering of the trap fabric, I would want to closely hand inspect. Half the value of these caps is about original application of the trap. In the skull shown, one can't see the nasal septum and there is puckering in other places.
