Opinions on SS FM pin and a possible rare receipt

Opinions on this nice condition SS FM pin.

With the pin there came this posssible ultra rare receipt?

Also if some one can help reading the handwriting, it would be a great help.
I am not good with this kind of writing.

The paper is a preliminary Ausweis, given for recognition of the first payment of dues prior to receipt of the small black F.M. Mitglieds Booklet. It's not a receipt for presentation of the pin. So I guess it's as rare as the member's book? Looks like the recipient was from Rüsselsheim, in the Rhein-Main area. The unit was Sturm 3 of Sturmbann I/ Standart 33.
