You can tell it's Italian because of the victory wreath on the skull's head.

It has nothing to do with the SS. It is an Italian Arditi Medal . They were made to Commerate members who fought in WW1 as part of the Arditi(Shock Troops). There are several models made during and after WW1 . I would sumise that the one shown was made after WW2 maybe for veterans or the collectors market. John Deana has a uniform of a WW1 Arditi on the International table for sale.
