The obverse on the 4th. medal is upside down and the suspension ring has been converted (poorly) from circular to teardrop.
I don't like the 3rd. one either (what could be a filed-down jump ring & soldering in wrong place on the suspension loop)

Curt, medal 3 IMO could be a P&L example. But the pics are not helping, more pics will make all clear.
