Medal of merit of vaccination's diffusion "OB INSTITIONEM VACCINAM LATE PROPAGANDAM" " instituited on the 20th March 1879
King Umberto I of Savoy awarded to the vaccinatores and health officers who had merits of vaccinations diffusions normally not
have the ring suspension (unportable) sometimes was assembled with ring suspension and ribbon.; awarded in gold and silver not
common; rare but not researched by collectors.

"six italians, dressed in rather unusual diving suits and equipped with materials of laughably little cost have swung the military balance of power in the Mediterranean in favour of the Axis".

      that what I found :
the medal was obviously issued during the kingdom of Umberto I (king of Italy ) so between 1878-1900.
iIt was awarded to doctors or period "healtcare assistant"( in this case Mr. RUFO NICOLA) who gave his "contribute to carry out the vaccination" free translation of the latin word " ob insitionem vaccinam late propagatamit" .
In this case was the vaccination against the Smallpox.
It is for sure in silver, I have no idea if it is rare ,probably not due to the smallpox vaccination last out till 1920 so more or less 50 years , and the value is probably the silver itself....
      ciao
      Andrea
