It's not a medal in a strict sense. It seems a plate belonging to the diverse array of souvenir trinkets sold in the shops and on board of the same military unit. They existed in many types (prayers sailor, medals with pictures of the unit, booklets with the name of the ship, keychains, etc..).

The official generic commemorative medal of the unit was this:
