this is a Fascist medal for families with many children,
unfortunately without ribbon.
On the ribbon one flakes=one child : there are also medals with 8 / 9 flakes

"Medaglia d'onore per le madri di famiglie numerose (Medal of honour for the mothers of large families)
It was instituted by Act no. 917 of the 22nd of May 1939 and awarded to the mothers of large families, who had at least seven living children. The ribbon of this medal was worn in the shape of a V, if the number of bows was in excess of ten. There is also a version with peg-connection, marked 38MI and produced by the firm Emilio Pagani using a non-compliant metal, and an enameled metal badge of the Fascist Union for Large Families that was coined by the firm Ventrella in Rome."

One bow/ribbon is not 1 child, but 8 living children.
It is a quit common medal. Value around 60 euro (in good/complete condition).
Below is my piece. It has a faded ribbon as the base should be green/blue.
