What you have here is an Early Heer NCO Medical Shoulder Board. 100% Original.

Since it is all felt material and the so-called "basket weave" piping it would be a private purchase shoulder strap, not necessarily early war.

I wonder if he was attached to 5 Panzer Regiment ?

The 5th Panzer seemed to wear the "5" on their shoulder boards. In some case even in Afrika. EM's would often pin a metal 5 on their boards along side NCO's and officers who wore metal 5's.

I realise its a medical board but he could be a 5 Panzer medic NCO trained to very high level in medical school,

I agree with Pauke;
private purchase, not necessarily "early".
In fact, the "5" appears to be later manufacture.
Medical straps / boards occasionally have their unit number
rather than the more common Aesculapius emblem
(staff & snake).
