I like the cap. My feeling is that the piping shade is probably for supply or transport rather than medical. The visor construction is unlike any that I have seen before but as I have said before it will probably take 100 more years for even the surviving variations to become fully known.
It has the alkor or paper composition sweatband which I have seen on a number of real alter art caps...some say this was not used due to lack of ruggedness but evidence does not support that belief.
I would call this a pre-war or early wartime crusher which probably had an almost patent leather like top side of the visor when newer,
