"Unser fuhrer" medal

I've noticed this medal on the polish auction site, and as I have never seen it before, I thoght I would ask you about it (not sure if this is the correct section for it). Is it real or just a fantasy piece, and if real, what actually is it? Some kind of rememberance piece, like "tag der arbeit" badges?
