The second ribbon on both nurses's ribbon bars is the Italian North Africa Medal-- equal stripes of black/white/red/white/green.
I can't tell from the glare what the first one is--it could be a Volkspflege award. Don't get anything out of the under the lapel ribbon on the right hand nurse. She appears to be wearing the "Afrika" cuff title on her left sleeve?
And is that not a German paratrooper on guard back there on the left, also in tropical uniform-- Italy 1943?Comment

famous ek picture
Hello Seba,
Yeps that is a famous one , it shows 2 red cross sisters who where among the rare female ek awards , namely ( the left one ) is DRK sister Hanny Weber and the right one is DRK sister Geolinde Münche .
* Hanny weber : she was awarded the ek 2 nd class fo her brave behavious in taken care of wounded soldiers during a Britisch bombing raid on the Afrika theater ( el - alamaein stellung )
* Geolinde Münche : she was awarded the ek for the same action as Hanny Weber , theyw here both in the same unit active.
Both sisters are awarded the cufftitle " Afrika " and as you mentioned this is a PK shot , meaning that on the uniform if Hanny Weber she is wearing the EK 2 trhough a button hole while she already wears the EK ribbon on the smal ribbon bar , her second award on this ribbon bar is the italian - German Campaign medal , this picture is widely used for propaganda purpose.
There where quite a few other females awarded this high bravery award ( and Hanna Reitsch even reached the iron cross 1 e class !! )
