I have something similar but can't think where it is at the moment but mine was given to a HJ member when he committed himself the Führer. It also came with an oath of allegiance to the Führer.

It is signed by Hauptbannführer Weber. This is a rank in the adult section of the HJ.

It is the commitment of Trude Werkmeister as HJ (Hitler Jugend) member on Adolf Hitler.

The date is interesting: 25 March 1945 only a few days before the end of TR.
