August 19, 1934 Fuhrer Ballot

I picked up this ballot that made AH the Fuhrer. I don't know if anyone has seen one of these. The back is plain and it is on the same thin paper as most ballots that I have seen. The red arrow, the pre-printed "X" in the Ja box and the statement below it make me think that this was some kind of documentation of the election.

The ballot on the bottom is labelled, Rotationsbrud: Berg & Otto. Hbg. 8 Gr. Reichenftr. 53/65. I don't know what this means.

Can anyone help identify what this may be?
