Need help with 3 docs - HJ, WP, Reichsbahn

Here are 3 docs I need help with.

1 - HJ Award Doc - what's it for??

2 - This doc was in the back of a WP I bought, didn't notice it until recently, and don't know what it is.

3 - Reichsbahn doc with a great picture. I've not seen one like this before, what is it??

2) A medical report of an Reichsbahn/State Railroad doctor giving different results of his
medical examinations of Fritz Kasten, born 4.6.1913 in Vossberg.
His profession: blacksmith

Any thought on the 2nd doc??? "Erflarung"?????

The "Erklärung" (statement) tells that the soldier declared/certified by his signature under

(death) penality that he is not of Jewish origin.

Very interesting, I've not seen one of these before....is it a common document for Heer soldiers to sign?
