Picked this up recently and in searching the forum, couldn't find another example. In fact, a search of the web only yields a couple of results (one on AHF and another, from the Cowdery Collection). I presume they are quite scarce.

Anyone else have one?

I have one issued in Berlin. These were voluntary and you had to pay a fee. That may be why you don't see them often.
