9 inch by 7 inch portrait, it appears that the officer has two scars under his left eye, and another to the right of his lips. No wound badge is visible , it could be a little lower than the general assault badge .

The studio name is on the lower left corner: Hanna Kunsch, Gottingen

You sure that's pionier black on his shoulderstraps? Looks like the device could be a medical one, so the waffenfarbe could be blue. ie he's an oberarzt.

The scars could well be duelling scars.

Odd, it looks like his Waffenfarbe are different colours. Compare the piping on his epaulettes with the darker colour on his collar tabs.

I'd also say they are duelling scars.

I don't believe he is an Oberarzt as I see three separate devices on his shoulder board and none of them appear to be a medical device. Plus he was awarded a GAB which would be a very uncommon award for a Doctor. I would dare say this officer is a Pioneer with dueling scars.
