help needed : Russian translation

Doesn't help at all that the entire part filled in on the person is obliterated!

It is a POW Camp "Certificate" that the Totally Obliterated Person is released from the camp in the city of Gori on 24 August 1945 to someplace that is an acronym (???) I cannot translate-- "Itel'sf" (possibly those are not quotation marks on the original but mistruck letters on the mimeograph copy) with further filled in data obscured.

It is signed by the Commandant, a Lieutenant Colonel, and the Director of (Re- one would assume)Education, a Guards Captain.

The first and second blanks are the person's name, the third is their nationality and the fourth and fifth are where they are going to - their place of residence (mesto zhitel'stva). The person was born in 1920.
