It appears to be an initial questionaire to practice a healing trade. In Warsaw, near as I can tell. And I think it says he's a technical dentist, or something similar. Whether it's real, or somebody just filled out a blank certificate (as indicated by missing stamp/signature at bottom) I have no idea.
