Opinion: urkunde Verwundete abz. in schwarz. Issued at Utrecht Holland

Hi guy's
I bought this Urkunde of the wound badge in black.
As such...nothing special but this one has been issued at Utrecht-Holland.
First of all I cannot identify the "truppenteil".
At the bottom it says "Krieg lazarett G/686/1"
I tried to find this number but I found 3 places where this one was , Utrecht , Almelo and even Arnhem.
Can anybody provide me more details ??
