I possess an Ostmedaille Award Document, I have seen others which are stamped with the eagle and swastika, but on mine there is no such stamp. Did this occur often? The paranoid side of me suggests that it could be forged though this seems unlikely for such a mass produced award. Any suggestions would be greatly appreciated.

Was the document issued by a hospital (Lazarett)? Or signed by a Doctor (Stabsarzt, Oberstabsarzt etc)?
I have a few examples where the stamps are missing when it was issued to the guy whilst in Military or Civil hospital...
What is the date of your award and unit he belonged to?

Inspired logic, I am impressed!
It was awarded to Sanitatsobersoldat Edgar Kreif on 6 August 1942 signed by the Stabsarzt und Kopaniechef whose signature is hard to distinguish. Would the stamp be missing simply because of the circumstances of the document being completed ie as you say a field hospital or such like?

I think it was due to being forgetful or as you say circumstances. the reason I mentioned Lazarett was because as mentioned they were the only ones that I had come across in this manner.
Do you have other documents to this person verifying his units or is this the only document?
