hi all
was given a load of badges and stuff off a vet that i met i posted them a couple of months ago, anyway when i got home today i had a letter off him saying he found this at home when he was clearing stuff away he brought this home with the other items but forgotten all about it and thought i would like it,so my question
1.what is it
2.what does it say
any help will be gratefully appreciated

not that common to find and a nice document you got there .... of course, it would be more interested to have one that was filled in and used but nothing you can do about that now...Cheers, Torsten.
