Help on a HJ Trade Competition Doc.

Can anyone help me on this document. As near as I can figure out it was for participating in a trade competition. It is 8 1/4" by just under 6". The German - US dictionaries suck. Any help would be appreciated as I would like to post it on E-stand with the correct description.
