Panzerjager Abteilung 52 - Award Document (Black Wound Badge)

Attached are photos of the award document for a black wound badge. I purchased the document at a local gunshow about 5 years ago.

I would appreciate any information about the document and/or the soldier and his unit (as well as any tips for further research into the soldier or his unit). Would this document have been prepared "in the field"? I notice that it is signed by a Hauptmann who was the wounded soldier's "Abt. Kommandeur". I am also very interested in learning whether or not this soldier survived WWII.

As you will see in the photos, the document came in a plastic slip cover marked "Made in W. Germany". Also, the document has what I believe are price markings handwritten on the reverse side, in the manner you see prices handwritten in Germany. Thus, I suspect that the document may have been purchased at a document shop, flea market, military fair, etc. in Germany and has since made its way here to the U.S.

I am not a document guy, so any observations would be most appreciated.
