The medal itself looks good, the ribbon is real but at 12mm is probably for a ribbon bar or a miniature.

Looks correct to me! Given the size/ratio of medal & eagle vs ribbon the ribbon shown here is definitely a 25mm ribbon (not 12mm) which was often used on shorter medal bars (instead of the 15mm width) but also on drapery mounts singles & on long medal bars like the 30mm version. Attached are 2 Whm 4 years singles; the left one is mounted on the narrower 25mm ribbon and the right one on 30mm ribbon & you encounter this configuration for most medals (EKs, KvKs, Flower campaigns etc)
