The document and the medal look fine me. The ribbon is too thin (perhaps a good one but for a feldspange).

Ian, I believe you're correct that it's for a miniature. The ribbon is roughly half the size of the medal and a 25mm ribbon bar would equal a 50mm medal, which is supposed to be 33mm.
