The enamelled medal is the Volkspflege award 3rd Class where-as the round medal is just the Volkspflege 'service' Medal ... i.e. the bottom award of the Social Welfare series. (2nd Class was an enamelled pin back and 1st Class a kneck award)

Ok. I dont speak german thx 'google traduction' but the anwer is here :
Ab dem 30. Januar 1942 konnte die III. Stufe sowie die Medaille für Verdienste im Krieg auch mit gekreuzten Schwertern auf dem Band verliehen werden. Auf den anderen Stufen war lt. Verordnung des Oberkommandos des Heeres das zusätzliche Tragen "mit Schwertern" nicht gestattet.
