Could someone outline the Third Reich's rules for replacing state long service ribbons with equivalent TR ones? Still don't really have a clear picture how that worked...

Only applied to WEHRMACHT personnel.
First phase: with creation of these awards in 1936, any serving member of the Wehrmacht with the earlier individual Imperial States' long service awards received the new ones. So a senior officer with a 1920 "XXV" got a document and the awards to wear thereafter as a 25 and 12.
Second phase: holders of the previous Imperial awards, if recalled for WEHRMACHT service were required (and we have seen many exceptions and period mistakes in the forums!!!) to replace them with the 1936 version, so an ex-NCO with a 1920 "XII" retreaded for WW2 wore a 12 and 4, or an old recalled officer swapped his "XXV" for a 25 and 12.
That applied ONLY in the Wehrmacht. Members of the Police, RAD, etc just continued on wearing their original Imperial awards.
Situation with the Waffen SS I'm less sure of, since about the only example I'm aware of was Sepp Dietrich-- and he "did his own thing" for wear of many awards. But he did replace his old Bavarian army long service with Wehrmacht ones. I suspect (but have seen NO documentation) that the Waffen SS was NOT considered "Wehrmacht" for the purposes of these awards-- at least not OFFICIALLY. -

Military (double dipping) and government employed time: so a 1914 Volunteer who stayed in the Provisional Reichsheer until 1920, receiving a "XII" military long service... got his 25 Years Treudienst in 1939-- counting that military time AGAIN as "government service."
BUT: discharged military personnel who served in the Weimar Civil service or Police (if discharged before 1935*) 1920-35 but went back into the Wehrmacht as (E) personnel did NOT get to count one day of THOSE years towards their Wehrmacht "long service."
* Weirdly enough, if a Policeman was still active when transferred over to the Wehrmacht in 1935, all his police time counted, and he was given the same rank in the army or air force. BUT say someone had retired in 1932... and went back as an (E) oficer in 1934-- that person reverted to his 1920 discharge seniority, so a Polizei-Oberstleutnant aD, say, was back again as a Hauptmann (E) with NO credit for service or rank 1920-32!!!!
