Help needed-- GWB entry + translation

Hi guys,

Just got this very nice soldbuch from the E-stand.
I've never handled such a long injury list before so help is very much appreciated.
This soldier was awarded the GWB on 4-9-'44 and, since he wasn't awarded the SWB, that would mean he lost at least two of his limbs on 10-8-'44.
Every entry on this page from then on are about this same injury, right?

I have trouble reading/translating the entries and I would love some help.

Very sad soldbuch knowing it was there at these dates..
