    I knew that handwriting wasn't new to my eyes.
    The entry on my Polizei-Soldbuch was made just 2 days after yours.
    In 1945 Fsch.Lazarett 21 was located in Tarvis, north-eastern Italy: the SSEuAB7 was retreating from Gradisca along the Isonzo valley during the last days of april 1945, so your man was wounded around that area.
    Have you got the date he was wounded?

        Congrats Jeff for this nice Addition to your Collection ! Great Pic, indeed !

        Concerning the Name "KOLOVRAT", it 's rare, but well, in my database from 2009, I found 16 People in Germany wearing this Name. 12 have german sounding first Name, and 4 has Vorname from southern Europe, these are : BLAZENKA - GORAN - MATO - VINKO. Fore sure yours was a Volksdeutsche from Balkan.

        It seems to me he himself do not sign "Heinrich" but more "Hendrik" or something like that, hard to say with these Scans.

              Disregard my above comment.

              I can read it better now - It says:

              Zerfetzung des linken Unterarms u.l. or u.b.(und linken or beider) Füße (Fußes). = Avulsion of left forearm and left foot (or feet).

              On May 2, 1945 he was admitted to the Field Hospital with his left forearm and foot (feet) blown to pieces.

              An even larger scan can bring certainty.

                  do you know the date he was wounded?
                  any other page to post? Bandenkampftagenliste maybe?
                  he sure has seen some fierce fighting over there and the were heavily bombed by allied air force during the retreat hence the huge wounds.
                  My man was also brutally wounded by plane bombs.

                        Looks like "u.d. re. Füße" (and the right foot) to me.

                          The scans you post are so small is pretty difficult to work with.
                          Would you be so kind to post a close ups pf the equipment page?

                          I'm a bit puzzled: seems like the man was first in some other Prinz Eugen unit such as Rgt.14? and then to the SS-Geb.-Aus.u.Ers.Btl.7, which is a nonsense to my eyes, but there wasn't any other Prinz Eugen in the area which he was killed.
                          Would you tell us you opinion regarding the unit he was in the day he was killed (a signature, a FPN)?
                          Maybe you can make it out better, since you have it in hand.

                              Can you see what's the line 4\5) entry?
                              Seems to me he went to the training unit after a long time with the Rgt.14, and that's nonsense to me but I may be wrong.
                              Maybe he was a trainer?

                                When they renumbered the Waffen SS Rgts in Okt. 43, the Rgt. 2 became Rgt. 14. The SS Gebirgsjäger Ausb. u. Ers Btl 7 was transfered to Gradiska/Oberitalien in 5.44???
