This is a medical code that would be found in a diagnosis conversion table. I am unaware what 42a specifically is. This was in hospital the next to last column is for comments in relation to discharge from the hospital.
I am unfamiliar with the code listed.
Hopefully others can help. Sorry, if I provided you with the same information you probably already had.

I have a medical defect table that lists Number 42 as Erkrankungen des Kehlkopfes (Diseases of the larynx) but I don't know how much the Kriegsmarine followed this table or had their own one with various differences, so it might be of help but then again it might not!
