Cholm shield entry in Wehrpass or Soldbuch ?

Does anyone ever have seen a original Cholmshield entry in either a Wehrpass or a Soldbuch ?

Iff you have one please show it here .

Entry for the Cholm Shield.
This particular example is taken from the Wehrpaß of an Oberleutnant with Inf.Rgt. 386, 218 Inf.Div., later Staffelführer with an Ost-Reiter Abtlg. and lastly as Hauptmann und National Sozialist Führungs Offizier of XXV Armee Korps, Festung Lorient.
