Replacing lost soldbuch

My question is if a soldier lost his soldbuch and it was replaced, is there location in the soldbuch where this would be documented? And what would be written in the soldbuch to indicate this? What information would they put in the new soldbuch?

I assume they would not rewrite all of his past ersatz units and would only log information that was current.

There should be a notation on the top of the first page indicating either Ersatzt, zweitschrift, or 2 ausfertigung. By regulation there should also be an unit ink stamped by the notation but this was often left out. It should list just his currant rank, all his awards lined up one per line and signed after the last, listed on page 22, any leaves in the last year and most of his hospital stays. However, like all paperwork there are variations.
