        Would not a slightly flawed A Type S&L be an RK that would had been produce several years earlier, than the June 44 date of Hermes award time ?

          Even if it was produced a couple years earlier, it might have been the last one in supply, or at the awards shop.

            It depends on the number / kind of wounds you received before loosing a limb. If none, you "just" get the silver grade. Gold for loss of 2 arms or 2 legs or 1 leg + 1 arm.
