It is always easier when the kanji are not posted sideways. The kanji are read top to bottom with the top being towards the point and the bottom the butt of the tang. Vertical not horizontal.

He received his swordsmith license in S18.10.20 [20 Oct 1943]
And his civilian name was
戸松直太郎
