Top signature ... that's tough. Looks like his first name might be 十五郎 Jūgorō; the second kanji of the surname might be 和.
Below the top signature, in darker ink and a different pen (so it appears to me) is the name 谷口君 Taniguchi-kun. Taniguchi [Valley Mouth] is a family name.

She was wondering why the date was so large and the name small -- that is opposite what one would expect UNLESS it was an important date like a graduation or something. Since S. 15 was the beginning of the Pacific War [not 2nd China-Japan War], it was probably his entry into the army -- and an important date to him.
