My guess was collar tab with something attached, but... came with a Japanese lot along with a few American items

It's a Japanese army pocket rank tab. The top part is normally the unit symbol with the rank (lance corporal) attached at the bottom. These are attached by button or safety pin on the tropical shirt pockets. Yours looks to have been attached to the button from the tabs on top. Great find.
