Translation Help with Document

Aloha, I found this document with some other Japanese WWII documents and was hoping that someone could tell me what it is or says. Any assistants will be greatly appreciated. Mahalo, David.

I can read everything on it, but still I don't know what it's for. It is a "question ticket" issued by the Tokyo (land) Survey Technonolgy Research Institute. The only logical explaintion is that you need to hand in this ticket to ask a question, but what kind of situation would require that kind of organization I don't know.
Anyway nothing military and no way to tell whether it's wartime.
