They are both the same kanji. The top one is the archaic version.

Yes, gyokuhō. I just checked Google to make certain and that is correct. Saw a sushiya-san named "Gyokuhō" [Valuable Jewel]

玉 translates as "jewel", but in translations I've seen, they used jade.

玉 gyoku is also part of the name used by Japanese for the "Banzai charge" Gyokusai [shattered jade]:
