Japanese Inscription/Translation Needed

This wool felt pennant is from the famous USA Brigadier General Albin F. Irzyk's estate. I know it's from an physical fitness/athletics school but would really like to know what the inscription reads.

That green wording means Physical education school.
That small kanji with blue background means Physical
1. Bare hand combat
6. Those kanji are too scripts and I can not read, but the last two Kanji mean to remember.

5. 1st class army sergeant/ 3rd class army sergeant..name.....
