It is the merit medal of the Italian branch of the St. Johns Ambulance.
I doubt that.
I checked my literature and IMHO the medal shown is a
Silberne Verdienstmedaille (silver merit medal) of the Order of Malta.
It is NOT the international merit order of the order of malta that was founded in 1920 (as shown in my link above).
It is the silver merit medal of the order of malta austrian branch that was created in 1916 during WW1 and was given till 1918 mainly to austrians and germans.
You can also see that the ribbon fits to the silbernes ehrenzeichen of the order of malta (austrian branch), that was also issued from 1916 to 1918. Two kind of ribbons are known: black and black with red-white edge. One riibon stands for Kriegsverdienst (war merit) but i don't know which.
Have now found a reference for this medal as haynau has so
graciously informed us, and the all black ribbon is for non combat service, while the ribbon with the red and white edge is for combattants.
