It is the Commemorative Medal for the Albanian Campaign, or Albanian Retreat medal. It was instituted on 5 April 1920 by the Prince-Regent Aleksandar (Alexander), future King Aleksandar I.

Weird thing, of pretty much every one of these I've seen with an original ribbon, it has frayed in exactly the same way. Also odd in that it is usually seen like this rather than on a trifold, like most Serbian/Yugoslavian decorations.
